Nurse Case Management

Case Management is a process that looks at the personal health needs of a patient diagnosed with a chornic illness, chronic long-term disease/condition, cancer, catastrophic diagnosis or complex health needs.  It is a process in which a professionally licensed provider (usually a nurse or social worker) asseses, plans and coordinates with the patient, doctors, family and other health care members working together with the goal of meeting an individual's unique health care needs.
